Mental Health Nurse Practitioner

The role of our Mental Health Nurse Practitioner is to support patients within our locality with acute and complex mental health matters in primary care across our 6 practices. They work with patients to risk assess and signpost, as well as review medication and initiate where this has been clinically identified. Patients can be booked in with our Mental Health Nurse Practitioner by contacting reception or via a GP/Duty referral. Our Mental Health Nurse can assess and refer to other suitable services where this is indicated and is able to provide education around self-management to support patients in managing independently in the community.

This role within our PCN offers patients an enhanced and specialist practitioner for mental health needs and ensures that timely and effective advice and guidance is given in relation to treatment options for mental health difficulties. North Lincolnshire South PCN is passionate about ensuring there is access to meaningful mental health support for the patient population in North Lincolnshire.
