What does the Social Prescribing Service do?

Once we have received your referral, your Social Prescribing Link Worker will get in touch with you within 5 working days to arrange an appointment.

Your Link Worker will listen to you and make sure they understand what’s going on in your life. They will work with you to develop a personalised plan based on what matters to you - whether that’s loneliness after a bereavement or feeling overwhelmed with money worries. They will then “link” you to the information, services, organisations and support that can help. Our service is part of the NHS and is free.

Our practical and emotional support has helped hundreds of people increase their confidence and improve their health and wellbeing by connecting people with the right support.

What is Social Prescribing?

Social prescribing tackles the non-medical problems that can affect our health and wellbeing. It connects people to the information, services, organisations and support to improve their health and wellbeing.

Many things affect our health and wellbeing that can’t be treated by doctors or medicine alone. Like loneliness, stress, money worries or poor housing.

It can help people to find the support they need, based on their unique situation.

Access the Service

You can access the service by either of the following options;

1) Contact your GP practice and ask to be referred to the social prescribers

2) (Over 18s only) Email hnyicb-nl.southpcn-referrals@nhs.net stating your: Registered GP Practice, Full Name, Date of Birth (dd/mm/yy), Preferred Contact Mobile/Email, NHS Number (if known) and a very brief outline of what support you need. 

3) Fill out the tear off slip in our leaflet and post it back to us.
